To start the Grievance Process, download the Grievance Intake form below, summarize the issue with clear and precise details, include dates as appropriate, and enter your personal email and phone number. Also submit your evidence documentation with the intake form. Once it is complete email the form and all evidence documentation to [email protected].

Note: You MUST enter your name, personal email, and personal phone number or your form will be delayed. Submitting the Form to us does not mean that you have a grievance. We will review the information on the grievance intake form and review your evidence to determine if you have a valid grievance case. Either way, a Union steward will be assigned to call you to discuss your issue. If it is determined that you have a grievance, the Union will proceed work with you to file an official grievance case to the agency on your behalf.
